IMPORTANT NOTICE
New COVID-19 Vaccination Requirement
Effective October 1st, 2021
The USCIS and CDC has updated vaccination requirements for the USCIS Immigration Medical Exam (Green Card Medical Exam)
The new requirement will require all Green Card applicants to be
fully vaccinated against COVID-19.
Currently, per USCIS and CDC guidelines, only the following COVID-19 vaccine can be accepted for the USCIS Immigration Medical Exam:
1. Pfizer-BioNTech COVID-19 Vaccine
Two Shots of Total, 21 days in between
- or -
2. Moderna COVID-19 Vaccine
Two Shots of Total, 28 days in between
- or -
3. Janssen COVID-19 Vaccine
Only require 1 Shot
Waivers for the COVID-19 vaccine is available under special circumstances,
please consult with your immigration attorney for details.

Dr. Ann Kim is authorized civil surgeon by U.S. Citizenship and Immigration Services (USCIS) to perform medical exam I-693 required for most Green Card applicants. (You do not need to print/bring I-693 form.)

It can take up 6 business days to complete the medical exam,
to receive the completed I-693 form in a sealed envelope.
For the first visit, please bring:
Government-issued photo identification (such as a valid passport or driver’s license)
Vaccination or immunization record:
- For adults TDAP, MMR, Varicella and Influenza vaccines (Oct - Mar)
- For applicants < 18 years old, bring all immunization records (such as DT, DTP, DTaP, Td, Tdap, OPV, IPV, MMR, Hib, hepatitis A, hepatitis B, varicella, pneumococcal influenza, rotavirus and meningococcal disease)
- If you had chickenpox as a child, please bring documentation.
Medical insurance card (for vaccination)
Payment in check or cash ($150, for adults, and $125 for kids younger than 15 ,for physical exam which includes documentation, exam, pdf copy of your I-693 form)
For applicants without insurance, additional payment will apply for vaccines,
which will be determined at the time of visit.
For ALL applicants, insurance will not likely cover the labs done for immigration purpose. We have a contracted lower price for labs but it must be paid at time of first visit, either as cash, check, credit card)
If you have no health insurance, you can get vaccinations at your local/nearby health centers for vaccinations ($10-$20 for each vaccine) or
it can be given in our office

- Syphilis (RPR) + gonorrhea + Quantiferon: $110
- Quantiferon only $75;
- Syphilis (RPR) only: $ 10
- Gonorrhea only: $ 30
- Varicella titer: $15
- MMR titer: $30
- Hepatitis B titer: $10
- Hepatitis A titer: $15
- Mandatory Blood draw fee: $6
If you have no health insurance, you can get vaccinations at your local/nearby health centers for vaccinations ($10-$20 for each vaccine) or
