​Referral Request

    Referral: For HMO plans, electronic referral is to be requested and obtain during an office visit. Please provide us with specialist name, office address, fax number, NPI number below before your visit so we can get it ready at the time of your visit. 
    If you've let Dr. Kim know about needing a referral in the most recent visit, please allow up to 1 - 2 business days to do the referral and will email it to you.
