It can take up 6 business days to complete the medical exam, to receive the completed I-693 form in a sealed envelope. 

For the first visit, please bring: 

• Government-issued photo identification (such as a valid passport)

• Vaccination or immunization record:

  • For adults

    • Covid vaccine ( received in 2023-2024)

    • TDAP, Tdap, Tetanus vaccine 

    • MMR vaccine

    • Varicella vaccine

    • Polio vaccine (requirement starting 5/1/2024)

    • Influenza vaccines (Oct 1st - Mar 31st)  

 

For applicants < 18 years old, bring all age appropriate immunization records (such as DT, DTP, DTaP, Td, Tdap, OPV, IPV, MMR, Hib, hepatitis A, hepatitis B, varicella, pneumococcal influenza, rotavirus and meningococcal disease)
         - If you had chickenpox as a child, please bring documentation.

Payment in cash ($150, for adults, and $125 for kids younger than 15)

For applicants without insurance, additional payment will apply for vaccines,which will be determined at the time of visit.

For ALL applicants, insurance does not cover the labs done for immigration purpose. We have a contracted lower price for labs but it must be paid at time of first visit, either as cash, check, credit card)
